Self pay costs:

-Initial consult $250

-Follow-ups $200

- There is a $30 travel fee for EACH home visit within a 30-minute radius of Shoreline.

- If you are located outside of the 30-minute radius, the travel fee is $50 for EACH home visit.

Payment is due at the time of service. Acceptable payments include cash, check, debit/credit, HSA & FSA, and Venmo.

Medicaid patients are offered a discounted price WITH proof of insurance.
